Use of *** was practiced in *** (environment) with *** cues required for patient to use correctly. Next we added auditory and visual stimuli by ***. In this environment, patient required *** cues to use external support correctly. We then attempted *** more times in this environment, and patient was able to use task support successfully with *** each time.
Over *** trials today, the average time to complete participation of *** using external cognitive support of *** was ***.

Generalization to real life: Home program was assigned to use task-specific support *** as practiced in speech therapy session. Patient / family / staff were educated about external cognitive support, which explicit situations to use, and how to complete log throughout the week for data.
Based on home program log, pt is using task-specific support accurately on *** / *** documented attempts.
