My Notepad

Today I’m sharing some free printables to use in speech therapy for people returning to work. Where do you start when jobs can be so different? For me, understanding personalized job needs and writing good goals is priority #1 for these patients! I’ve created a few SLP Interview Tools that help make this fact-finding process efficient and part of our first visit conversation. Today, you can print my SLP  Interview Tool for people going back to work and the first Home Program that I assign for those heading back to work. Need to see more? The Back To Work Series has it all.

SLP Patient Interview Tool: I print this out and use this at each initial evaluation for higher-level people returning to work. This helps me instantly focus therapy and goals through an activity/participation lens. I always note if productivity is a factor (then we track speed of completion for personalized tasks!) and if multi-tasking is needed (then I make sure to attempt multiple tasks at once or add in distractions!)

Home Assignment: Even though I have an idea of the job tasks that a person is responsible for after our first meeting, I still like to have someone complete a written description of their daily/weekly tasks. This helps me see what tasks are done most frequently, and it also helps me develop strategies particular to someone’s job setting or environment.

Using these tools should help you easily customize therapy and cognitive-communication goals to your Person’s unique job situation! If you want therapy ideas that directly relate to the interview tool above, be sure to check out the Back To Work Series!

If your patients aren’t returning to work, but they are returning to home activities, please see the Home Sweet Home Series as I’ve also created an Interview Tool and Home Program template specifically for those working on independence for home tasks.
